Recently I have been looking into the different types of providers in the area of women's health It has been interesting because there is the aspect of infant mortality rates in certain populations It seems the current system is hurting some groups more than others and midwives could be a solution I am trying to figure out which could be a better route OB/GYN, CNM, or CPM In the case of low-risk pregnancies it seems like there is a stigma surrounding CNMs and even worse CPMs, yet the overwhelming amount of people who share their experiences say that they felt they were not given as much attention as they would have liked from the OBs they have had experience with When thinking about the mortality rates of certain populations and how studies show that bias can play a role in a provider's response to a patient, I don't understand the stigma in regards to CNMs and more so CPMs when the overwhelming majority of experiences shared express that more personal attention was received Here's how I see it, we have a group of a population known as "group Z" Relatively speaking compared to other groups, group Z is having not so good birth outcomes Providers have been shown to have bias where they are less sensitive to the concerns of people in group Z As is, OB/GYNs do not provide personalized care in general due to seeing so many patients We have bias coupled with limited time spent/a lack of personalized care affecting group Z The statistics make it clear that something is wrong, yet people look down on CNMs and more so CPMs In the cases where proper training is received, CNMs and CPMs look like they could be a solution to this problem We can't overlook the mortality rates when it comes to minorities It seems as though the title of physician has people running to system where statistically speaking the odds are trash for certain groups I get that scope creep is a serious thing, but when we have the statistics that we do, I don't see nurse midwifes the same as nurse anesthetists I wish more people would talk about this The statistics that we have in regards to minority births are not normal Midwives may be a solution The more I learn, it seems like due to the patient ratio OBs just pop-in during births and cannot not give much personalized attention throughout the pregnancy That is not appealing, especially after hearing how births can be influenced by practices early on in the pregnancy, the ability to spot something early is important Education is important Midwives are known for walking alongside and educating families during a pregnancy If the system is not set up where OBs can give more time to patients, what good is that when it comes to decreasing infant mortality rates? I'm not trying to knock OBs, I just feel as though this is an area where mid-levels should not be overlooked and recognized On an global scale, midwifery is more established I don't think this is by chance, pregnant mothers need more individualized and personal attention throughout a pregnancy I've been thinking about this for a while and wanted to know if it could serve as possible future video topic? it's also important to note that ob/gyn has some absolutely horrific origins that continue to impact the specialty today the specialty faces issues with paternalism (particularly in regards to voluntary sterilization) and there's the MASSIVE problem of overlooking pain in gyn procedures brob/gyn is the only specialty in medicine where it's 100 normal to not give anesthesia or pain relief for often excruciating procedures like IUD placement/removals, pap smears, cervix biopsies, etc sociological issues like systemic racism show themselves HARD in ob/gyn, particularly in the us, where our maternal mortality rates are horrific and significantly worse for black women brmany ob/gyns are STILL operating under extremely outdated information (like that the cervix has no nerves) and it's very hard for patients to find providers that will give them pain relief measures that are even remotely appropriate to the pain of a procedure Comment from : Alix |
